Yeah, you're right. I misread your comment. You didn't say doctors don't get paid. But I don't understand how healthcare is a "privilege." Back in the Founders' Day, sure, healthcare was a privilege because they barely had a concept of health care. Today we understand that it's cruel to leave someone untreated. I'm not sure why you're calling it a "positive right"--because it costs money and requires someone else to step in? We don't leave someone hit by a car to die in the street without assistance because that's just unethical in a modern, civil society. How is that NOT a basic right?
